How long after exercise can rhabdomyolysis symptoms appear? Symptoms can begin within hours, but they may not become noticeable until a day or several days after the muscle injury. The CDC specifically says rhabdo symptoms may not begin until hours or days after the initial injury and, for some people, may take several days to appear. That's one reason exercise-induced rhabdomyolysis—also called exertional rhabdo—can be confusing.

You may finish a difficult workout feeling tired but otherwise okay. The next day you're sore and assume it's normal delayed-onset muscle soreness (DOMS). Then the pain, weakness, swelling, or other symptoms become much more noticeable.

So, feeling okay immediately after a workout doesn't necessarily rule out rhabdo.

There also isn't one exact rhabdo timeline that applies to everybody.

How Long After Exercise Can Rhabdo Symptoms Start?

The best answer is hours to days, with some people not recognizing symptoms until several days after the muscle injury.

Here's a practical way to think about the timing:

Important: The table isn't an official diagnostic timetable.

There is no official rule saying, for example, that symptoms appearing at 12 hours mean rhabdo while symptoms appearing at 48 hours mean DOMS.

The overlap is much greater than that.

Also Important:  The time it takes for muscle damage to occur, the time when you notice symptoms, and the time when your creatine kinase (CK) level reaches its highest point are not necessarily the same.

Can Rhabdo Show Up 2 Days After Exercise?

Yes.

A delay of 2 days does not rule out exertional rhabdomyolysis.

One published case involved a 12-year-old boy who developed intense thigh soreness and dark urine 2 days after exercise. That's one case, so it doesn't tell us how commonly this happens, but it does show that a 2-day delay is possible.

In my interview with Amy, who developed rhabdo following a CrossFit Murph workout, she said her main symptom was being unable to straighten her arms fully without significant pain. She reported that this didn't start until 2 days after the Murph workout. Her arms began swelling after she exercised again the following morning.

Amy's experience is not proof of what happens to everyone. It's a real-world example of why the absence of immediate symptoms doesn't rule out a problem.

Can Rhabdo Show Up 3 Days After Exercise?

Yes, that's possible too.

A published report involving a healthy 21-year-old woman described excessive muscle soreness and swelling that led her to seek medical attention 3 days after an unusually demanding biceps exercise session.

Again, this is case-report evidence. It shows what can happen, not what usually happens.

That's an important distinction because I don't think the research gives us a reliable percentage of exertional-rhabdo patients whose symptoms first appear at 24, 48, or 72 hours.

The stronger conclusion is simpler: Rhabdo symptoms can be delayed for days.

Why Can Rhabdomyolysis Symptoms Be Delayed?

Muscle injury and symptoms don't necessarily happen at exactly the same time.

When muscle cells are significantly damaged, they release substances that normally remain inside the cells, including CK, myoglobin, and electrolytes.

Those changes begin with the muscle injury, but pain, swelling, weakness, laboratory test abnormalities, and visible urine changes can evolve differently over time.

You can damage muscle during a workout without immediately realizing how significant the injury is.

Rhabdo vs. DOMS: Can You Tell the Difference?

This is where things get difficult.

Delayed-onset muscle soreness (DOMS) also occurs after unfamiliar or unusually strenuous exercise, especially exercise with a large eccentric—or muscle-lengthening—component.

DOMS usually doesn't hit its maximum intensity immediately. Soreness is often noticeable 1–3 days after the workout.

That overlaps with the period when exertional rhabdo may also become apparent.

This table may help you see how they compare:

There is one big caution with that table:

You can't accurately diagnose rhabdo versus DOMS yourself from a checklist.

Timing isn't enough. Absence of dark urine color isn't enough.

The CDC points out that rhabdo symptoms can look like other conditions. This is why a CK blood test is an important part of the diagnosis.

That's why I would avoid claims like: “If you're still sore after 48 hours, you have rhabdo.”

That's not true. Ordinary DOMS can still be quite uncomfortable then.

I'd also avoid the opposite assumption: “My urine looks normal, so it don't be rhabdo.”

That's not true either.

What Rhabdo Symptoms Should You Watch For?

The CDC highlights three main symptoms:

  • Muscle cramps, aches, or pain that are more severe than expected
  • Dark, tea- or cola-colored urine
  • Weakness or unusual tiredness, including difficulty doing physical tasks you could normally perform

Significant muscle swelling can also occur with rhabdomyolysis.

One thing I would pay attention to after a hard or unfamiliar workout is loss of normal function.

If your biceps are a little sore when you straighten your arm, that sounds very different from an arm that has become markedly swollen and difficult to straighten normally.

But even significant soreness, swelling, or limited range of motion doesn't diagnose rhabdo by itself.

Another Possible Symptom

Another clue I personally pay attention to is muscle pain while you're not moving.

With ordinary DOMS, soreness is often most noticeable when you move the muscle, stretch it, contract it, or press on it. In other words, you really don't feel pain when you are not moving.

Rhabdo pain can be much more severe and persistent. If a muscle is hurting badly even while you're sitting still, especially when there is significant swelling, weakness, or loss of normal function, that would make me more concerned.

Keep in mind, however, that pain at rest isn't a rhabdo test. For example, people with tennis elbow or arthritis may experience pain at rest too. Think of resting pain as another clue, not definitive proof of rhabdomyolysis.

If your biceps are a little sore when you straighten your arm, that sounds very different from an arm that has become markedly swollen and difficult to straighten normally.

But even significant soreness, swelling, or limited range of motion doesn't diagnose rhabdo by itself.

It's the unusual severity or combination of symptoms that should prompt medical evaluation rather than an attempt to diagnose yourself at home.

Does Rhabdo Always Cause Dark Urine?

No.

This is worth emphasizing because dark urine has become almost synonymous with rhabdo on the internet.

Dark tea- or cola-colored urine can occur when myoglobin released from damaged muscle reaches the urine. But rhabdomyolysis does not require visibly dark urine.

In one report that involved 136 people who had exercise-induced rhabdo, dark urine was only seen in 36.8% of the people. This tells us that not everyone who develops rhabdo gets the dark-urine symptom.

Myoglobin also disappears relatively quickly. A major clinical review reports that blood myoglobin rises rapidly after muscle injury and may return to normal within about 24 hours, while CK remains elevated much longer.

So remember this:

Normal-looking urine does not rule out rhabdomyolysis.

Don't wait for your urine to turn brown before taking unusually severe post-exercise symptoms seriously.

When Does CK Rise After Rhabdomyolysis?

Creatine kinase, usually shortened to CK, is an enzyme found in muscle.

When muscle cells are damaged, CK enters the bloodstream. Measuring CK is an important part of evaluating if you have rhabdomyolysis.

But CK doesn't instantly reach its highest level.

A major clinical review reported that CK generally begins rising about 2–12 hours after muscle injury, often reaches its peak around 3–5 days, and then declines over the following days if additional muscle injury is not occurring.

The CDC recommends repeat CK testing because the first result may be taken while CK is still rising.

This helps explain an otherwise confusing situation: Someone can already have muscle pain and swelling while their CK is still climbing.

A High CK Doesn't Tell the Whole Story

Strenuous exercise by itself can cause surprisingly large CK increases in healthy people. Research on exertional rhabdomyolysis has documented substantial variation in CK responses following strenuous exercise.

Something else: a higher CK does not always track with more severe symptoms.

For example, in this report, published in 2024, researchers noted that CK level by itself was not associated with who developed acute kidney injury.

That doesn't mean CK isn't important. It is. Rather, it means one number doesn't tell the whole story.

Symptoms and CK Don't Follow the Same Time Table

Imagine someone completes a very hard workout on Saturday.

  • Sunday they're sore.
  • Monday they have much more pain and noticeable swelling.
  • Tuesday, a doctor checks CK and finds a large elevation.
  • That doesn't mean the rhabdo suddenly began on Tuesday.

The muscle injury happened earlier. Symptoms became noticeable later. The blood test was performed later still.

Amy's experience illustrates this nicely. When she first went to the emergency room, she reported a CK of about 14,000 U/L. After getting IV fluids, it declined but then increased and reportedly peaked at about 21,000 U/L before declining.

That's one person's experience, not a universal rhabdo timeline.

But it illustrates an important point:

Symptom timing and CK timing aren't interchangeable.

Can You Have Rhabdo Without Severe Pain?

You sure can.

Not everybody with rhabdomyolysis has every classic symptom.

Pain, weakness, swelling and dark urine can occur in different combinations. That's another reason I wouldn't use any single symptom to rule rhabdo in or out.

What Makes Exercise-Induced Rhabdo More Likely?

Exercise-induced rhabdomyolysis is often associated with exercise that places unusually high demands on the muscles.

Recognized risk factors include prolonged exertion, high-intensity exercise, and heat exposure. Certain illnesses, medical conditions, medications and drugs can also increase risk.

Unaccustomed exercise deserves special attention too. Any activity you're not used to has the potential to trigger rhabdo if you do enough of it.

And being fit doesn't make you immune. Rhabdo doesn't discriminate.

For example, a marathon runner might have excellent aerobic fitness and still expose the arms to an enormous unfamiliar workload during a high-volume pull-up workout.

When Should You Seek Medical Attention?

While no article can diagnose rhabdomyolysis,  if you develop muscle pain that is much more severe than expected after exercise, especially with swelling, weakness, major loss of normal function, or dark urine, seek prompt medical evaluation.

A doctor may check CK and other laboratory measures and will probably repeat CK testing to see whether it is rising or falling.

And I'll repeat something because I think it's particularly important:

Don't wait for dark urine before getting checked.

Because you may never develop it that symptom.

Can Rhabdomyolysis Cause Long-Term Problems?

Most people recover from exercise-induced rhabdomyolysis, but some people continue to report problems long after the original episode.

Over the years, people have told me about profound fatigue, unusual electrical or nerve-like sensations in their arms or legs, and muscles that become painful or swollen again when they return to exercise. I've heard these complaints from some people 6–12 months or even longer after their rhabdo.

Research on long-term recovery from exertional rhabdomyolysis is surprisingly limited. However, a 2023 study found that some people continued to report muscle symptoms during exercise and at rest, fatigue, and other complaints 6 and 12 months after exertional rhabdomyolysis or exertional heat stroke. The study was small, particularly the rhabdo group, so we can't use it to say how common long-term symptoms are.

Persistent symptoms also shouldn't automatically be assumed to be a normal part of rhabdo recovery. Medical reviews recommend further evaluation when problems such as persistent weakness, exercise intolerance, recurrent episodes of rhabdo, or persistently elevated CK continue after the initial event. In some cases, rhabdomyolysis can uncover an underlying muscle or genetic disorder that wasn't previously recognized.

If you're still experiencing significant weakness, swelling, unusual muscle pain, or difficulty returning to exercise months after rhabdo, I think those symptoms deserve medical attention rather than simply being told to wait longer.

Why Exercise-Induced Rhabdo Is Easy to Miss

Exercise will make us sore sometimes.

That's part of the problem.

Do 50 bodyweight squats after not training your legs for several months and you expect to be sore tomorrow.

When that soreness becomes intense, it's easy to say:

“I really overdid it.”

But if the pain seems far beyond your normal response, swelling is significant, you're unusually weak, or you can't use the affected muscles normally, don't rely on the rhabdo-bookarticles and videos to tell you whether it's DOMS or rhabdo.

There isn't a magic hour when DOMS stops and rhabdo begins.

The Bottom Line

So, how long after exercise can rhabdomyolysis symptoms appear?

Rhabdo symptoms can begin within hours of muscle injury, but they may also not become noticeable until a day or several days later.  Some people have told me they began to feel sore almost immediately after they stopped working out.

That timeline overlaps substantially with ordinary muscle soreness, so timing alone cannot diagnose rhabdomyolysis or rule it out.

If post-workout pain, swelling, weakness, or loss of function seems unusually severe, don't wait for dark urine or a particular number of hours to pass before getting medical advice.

CK typically begins increasing within hours after muscle injury. One major clinical review reports a rise about 2–12 hours after injury and a peak around 3–5 days afterward. Individual timelines vary.

Can you have a mild case of rhabdo?

Rhabdomyolysis can vary greatly in severity, and some people recover without kidney problems or other serious complications. Still, I don't particularly like calling rhabdo “mild.” Rhabdomyolysis means muscle breakdown has gone beyond the normal response to exercise, so even a less severe case deserves to be taken seriously.

Fear of exercising again after getting rhabdo

There's another long-term effect of rhabdomyolysis that I don't think gets enough attention: fear of exercising again.

Over the years, many people who have contacted me after developing exercise-induced rhabdo have told me they were terrified to return to the gym. Terrified was the word they all used! They were really afraid rhabdo would return again.  Some people describe the experience as traumatic.

I understand why. Going from a workout to severe pain, an emergency room visit, getting IV fluids, and spending several days in the hospital, not to mention unexpected medical bills, can change the way someone thinks about exercise.

Surprisingly, there is very little research on this. A 2023 study did find anxiety, mental-health concerns, months after exertional rhabdomyolysis and exertional heat stroke, but the researchers did not specifically investigate post-traumatic stress disorder.

If fear is preventing someone from returning to normal activity long after they have medically recovered, that deserves attention too. Recovery from rhabdo isn't just about getting the CK level back to normal.

Can rhabdo happen after one workout?

Yes. Exertional rhabdomyolysis has been documented after single bouts of unusually strenuous or unfamiliar exercise, including high-volume resistance exercise. Most of the people who have reached out to me over the years told me they developed it after one workout.
